Benefit Reverification & Cancellation Policy

Benefit Reverification – All patients, new and existing, will have their benefits reverified and will be asked to complete our new intake forms. The front offices will need to make a copy of each patient’s insurance card(s) and driver’s license.  

Cancellation Policy – If a patient needs to cancel or reschedule an appointment, we request that it be done 24 hours in advance of the scheduled appointment.  If a cancellation occurs within less than 24 hours of their appointment or if the patient does not show up to their appointment, a cancellation fee of $25 will be applied. No fee will be applied for cancellations made with more than a 24-hour notice.
